We have powers to undertake ‘own initiative’ investigations where evidence suggests that there may be systemic service failure or maladministration.
Where we are satisfied that the criteria for investigation is met we may undertake two types of own initiative investigation:
- Extended Investigation – The extension of a matter already under investigation
- Wider Investigation – A stand-alone investigation which does not relate to a complaint made by an individual
