We have the power to undertake ‘own initiative’ investigations where evidence suggests that there may be systemic service failure or maladministration but where we have not received a complaint. 

We must be satisfied that the  criteria for investigation  is met. 

 We may start an own initiative investigation in 2 ways: 

  • Extending a current investigation into a complaint made by an individual  
  • Starting a stand-alone investigation which does not relate to a complaint made by an individual
