Miss X complained that Hywel Dda University Health Board (the Health Board) had failed to respond to the complaint she had submitted 13 months earlier.
The Health Board agreed to apologise for the delay, offer financial redress of £250, in recognition of the additional time and trouble to which Miss X had been put in pursuing the complaint, and issue its formal response to her complaint within 20 working days.
