Mrs A, on behalf of her mother, complained that her grandmother’s discharge from Morriston Hospital was unsafe.

The evidence suggested that the discharge was not unreasonable. However, the Ombudsman was concerned that the Occuptional Therapy (OT) Team was not aware of a home environmental issue which had been recorded in the notes by another member of staff immediately precedings its involvement in Mrs A’s grandmother’s care. He decided to settle the complaint without an investigation.

The Ombudsman sought Swansea Bay University Health Board’s agreement to, within 1 month, apologise to the family for failing to identify the issue and to reflect on the case to ensure that this type of information is noted during future OT assessments.
