Mrs A complained about the care and treatment provided to her by the Health Board in respect of the management of her pregnancy loss. Specifically, she said that the Health Board discarded tissue from the loss, which should have been sent for genetic testing. Additionally, Mrs A raised concerns about the handling of her complaint, in that it had been printed out and read by all staff.

The Ombudsman found that all Mrs A’s medical records and the majority of her nursing records were missing.

During the course of the Ombudsman’s consideration of Mrs A’s complaint, the Health Board requested to be provided with the opportunity to re-consider Mrs A’s complaint, including the loss of the clinical records, and the subsequent impact of this. The Ombudsman agreed for the Health Board to re-consider Mrs A’s complaint and to provide a response in accordance with The National Health Service (Concerns, Complaints and Redress Arrangements) (Wales) Regulations 2011 within 2 months.
