Ms X complained that the Health Board showed a lack of care and professionalism which she believed to have contributed to the death of her father.

While considering the information from both Ms X and the Health Board, it was noted that her concerns were not taken through the Putting Things Right process. The Ombudsman sought and obtained agreement from the Health Board that it carry out an investigation under the PTR process and issue its complaint response within 3 months.
