Ms A complained that in May 2021 she was not informed about the risk of losing her baby following a 13-week scan. She also complained about the lack of monitoring and internal communication between departments. Other issues complained about included the insensitively of a staff member when discussing an autopsy, and post miscarriage hospital waiting room arrangements.

The Ombudsman concluded that the Health Board had reasonably addressed many issues. However she was concerned that the apology provided for the accepted communication failings during the 13-week scan, did not fully address the matter. She decided to settle the complaint.

She sought and obtained the Health Board’s agreement to offer Ms A a financial redress payment of £1,000, in recognition of the significant distress and trauma caused to her and her family.
