Ms A complained about the care and treatment provided by Hywel Dda University Health Board’s (“the Health Board”) mental health team to her sister in law.
The assessment found that, although there had been a serious incident review, the Health Board had not issued a response under the Putting Things Right complaint regulations (“the Regulations”). The Ombudsman sought and gained the Health Board’s agreement to issue a full response to the complaint, which would comply with the regulations including timescales set out there.
