Ms Q complained about the care and treatment her late father, Mr T,
received between 14 and 20 October 2022 while at Morriston Hospital.
Specifically, we considered:

• The management of Mr T’s renal care, and whether any delays in
addressing his fluid overload and in commencing dialysis and
steroids impacted on his death.
• Whether Mr T’s nursing care was appropriate, and whether a leg
infection and sepsis were appropriately identified.
• Whether nursing staff should have identified Mr T’s deterioration
on 19 October.

The investigation found that the management of Mr T’s renal care was
appropriate and there were no delays in addressing his fluid overload
and commencing dialysis and steroids.

The investigation also found that Mr T’s nursing care was appropriate,
and signs of an infection were acted upon quickly. There was also no
evidence to show that nursing staff failed to identify Mr T’s deterioration
on 19 October.

The complaints were not upheld.
