Ms A complained about the standard of clinical care provided to her late grandmother, Mrs B, in June and July 2023 when she was admitted to Morriston Hospital with confusion and following a fall. In particular, Ms A raised concerns that evidence of a stroke had been missed during this period.
The Ombudsman obtained clinical advice on the complaint.

The investigation identified one shortcoming in the medical care; Mrs B should have been reviewed by the medical team for her identified low sodium levels before her discharge from the Emergency Department. However, it was noted that any investigations relating to this could have taken place on an outpatient basis and in any event Mrs B was reviewed by the medical team less than 2 days later. Therefore, this did not alter the treatment or outcome for Mrs B.

The Ombudsman found that there was no recorded clinical evidence of a stroke that had been missed by treating clinicians.

The Ombudsman did not uphold the complaint.
