Mrs A complained that Aneurin Bevan University Health Board had failed to conclude an investigation or provide a response to her about the care and treatment provided to her late husband, Mr A.

The Ombudsman found that the Health Board had commenced a Serious Incident Investigation in April 2024 and there had been a significant delay in the Health Board concluding the investigation and responding to Mrs A. The Ombudsman said this caused frustration to Mrs A and decided to settle the complaint without a formal investigation.

The Ombudsman sought and gained the Health Board’s agreement to apologise and explain the reasons for the delay, offer a redress payment of £250 to Mrs A in recognition of the delay, and to issue a complaint response to her compliant with the ‘Putting Things Right’ process within 8 weeks.
